Patient Paperwork

Please come to your first appointment with the new patient paperwork completed.

• New Patient/Returning Patient Paperwork
• Direct Access Form (required if you’re coming without a prescription)
• Notice of Privacy Practices - HIPPA Information

There may be 1-2 additional forms that we will have you complete in the office that are insurance or injury-specific.

If you don’t have access to a printer, that's not a problem. Stop by the office 20 minutes before your scheduled appointment time and you can complete it in person. Alternatively, you can complete the paperwork electronically and email it to us ahead of your appointment.
